Youth Permanency Connections (YPC)

Youth Permanency Connections program is designed to prevent youth from aging out of the foster care system without a significant adult connection. The service is available regardless of where the child is placed. Staff will work with the enrolled youth to identify and/or recruit a specific family or adult connection that they want to remain close and connected to throughout adulthood.

Our dedicated team of Social Workers meet with each youth to help identify, locate, and explore all potential individuals willing and able to make a commitment to a teen.

Services to the youth and the identified life-long adult connection continue until the relationship is safe, stable and secure and they have the skills and resources to assist them in the future.
