Empower 60+
Empower 60+ addresses the behavioral health and social needs of adults aged 60 and up.
Our Care Navigators will come to you and conduct comprehensive needs assessments. They will also provide personalized care coordination for seniors and their family caregivers, helping older adults access critical services including:
- behavioral health and medical care
- food assistance
- housing support
- and transportation
Older adults receive holistic, wraparound support tailored to their unique needs.
Empower 60+ connects older adults with mobile and virtual behavioral health services, offering timely support for depression, anxiety, grief, and adjustment challenges.
Care Navigators connect individuals to recovery coaches and peer specialists and will also facilitate access to in-home services.
Empower 60+ helps to:
Reduce social isolation
Enhance wellness
Prevent unnecessary hospitalizations or institutionalization
Promotes aging in place with dignity and connection

This service is available in Carver, Plymouth, Kingston and Duxbury through our CFS Community Behavioral Health Center.
